Terms of Service

1. Acceptance of Terms

By accessing or using the website at ormandandcook.com (the "Site"), scheduling services, or enrolling in any program offered by Ormand & Cook Wellness, PLLC ("Ormand & Cook Wellness," "we," "us," or "our"), you ("patient," "client," or "user") agree to be bound by these Terms of Service. If you do not agree, please do not use our Site or services.

2. Nature of Services

Ormand & Cook Wellness is a concierge wellness practice operated by licensed Family Nurse Practitioners (FNP-C) in North Carolina. Services include: the Metabolic Clarity Program (MCP), Hormone Clarity Program (HCP), GLP-1 protocols, NAD+ subcutaneous injection therapy, IV hydration and injection therapy, hormone replacement therapy (HRT), functional lab testing, and concierge/in-home acute care. Telehealth services are available statewide throughout North Carolina.

Our services do not replace emergency medical care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

3. Medical Disclaimer

Content on this Site, including program materials, blog posts, and educational resources, is for informational and educational purposes only. It does not constitute medical advice, diagnosis, or treatment. Always consult your qualified healthcare provider regarding any medical condition. Reliance on any information provided by Ormand & Cook Wellness is solely at your own risk.

Individual results from our programs will vary. Statements about outcomes (including weight loss, hormone optimization, or energy improvement) are not guarantees of results.

4. Eligibility

You must be at least 18 years of age to use our services and must be a resident of North Carolina. By using the Site or enrolling in any program, you represent that you meet these requirements and are legally capable of entering into binding contracts.

5. Appointments, Cancellations & Refunds

Appointments: Scheduling is confirmed upon receipt of required intake documentation and applicable fees.

Cancellations: Appointments may be cancelled up to 24 hours before the scheduled time. Cancellations made less than 24 hours in advance or no-shows may be subject to a cancellation fee. To cancel, contact us at OCwellness@ormandandcook.com.

No Refund Policy: All fees paid to Ormand & Cook Wellness are non-refundable. This applies to all services, programs, and products including but not limited to: program enrollment fees (MCP, HCP, and all bundled programs), IV hydration and injection therapy, NAD+ therapy, GLP-1 protocols, lab testing, and any other service rendered. By enrolling or purchasing, you acknowledge and agree that no refunds will be issued under any circumstances.

6. Patient Responsibilities

You agree to provide accurate, current, and complete health information during intake and throughout your care. Withholding or misrepresenting health information may compromise your safety. You are responsible for following the care recommendations provided by your Ormand & Cook Wellness practitioner.

7. Intellectual Property

All Site content, including program materials, educational resources, graphics, logos, and text, is the property of Ormand & Cook Wellness, PLLC and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works from any Site content without express written permission.

8. Limitation of Liability

To the fullest extent permitted by law, Ormand & Cook Wellness, PLLC, its owners, practitioners, and staff shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Site or services. Our total liability shall not exceed the fees paid by you for the specific service giving rise to the claim.

9. Governing Law

These Terms are governed by the laws of the State of North Carolina. Any disputes shall be resolved in the courts of Gaston County, North Carolina.

10. Changes to Terms

We reserve the right to update these Terms at any time. Changes will be posted on this page with an updated effective date. Continued use of the Site or services following any changes constitutes acceptance of the revised Terms.

Information you provide directly: When you complete intake forms, schedule services, enroll in programs, or contact us, we may collect: name, date of birth, contact information (email, phone, address), health history and symptom information, payment information, and other information you voluntarily submit.

Information collected automatically: When you visit our Site, we may collect standard web analytics data such as IP address, browser type, pages visited, and time on Site. We may use cookies to improve your experience. You may disable cookies through your browser settings.

2. How We Use Your Information

We use collected information to: provide and manage your care and program enrollment; communicate about appointments, results, and program updates; send educational or promotional communications (with your consent); comply with healthcare and legal obligations; and improve our website and services.

3. HIPAA & Protected Health Information

As a healthcare practice, we are subject to the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information (PHI) is handled in accordance with our Notice of Privacy Practices, available upon request. We will not sell, rent, or share your PHI with third parties for marketing purposes without your explicit written authorization.

With service providers: Third-party vendors assisting in practice operations — including Quest Diagnostics, ZRT Laboratory, Fullscript, electronic health platforms, and payment processors — under appropriate confidentiality agreements.

With your consent: For any other purpose with your explicit authorization.

Our services are not directed to individuals under 18. We do not knowingly collect personal information from minors. If you believe we have inadvertently collected such information, please contact us immediately.
